This study titled, "Treatment failures (revision or arthroplasty) after knee osteochondral allograft transplantation with minimum two-year follow-up”, investigates knee osteochondral allograft transplantation (OCAT) outcomes, treatment failures, and subsequent revision surgeries. Analyzing data from a lifelong outcomes registry, the research finds that OCAT generally leads to functional graft survival in the short to mid-term for a majority of cases (70-88%). However, treatment failures occur in 23.3% of cases, with variables such as allograft preservation method (standard preservation vs. MOPS®), patient age, BMI, and adherence to postoperative protocols significantly influencing outcomes. Revision surgeries post-OCAT show promising results, with 66% graft survival rates for at least 2 years, particularly when using MOPS allografts. The study underscores the importance of considering modifiable factors like preservation method and patient adherence in improving OCAT outcomes and suggests that revision OCAT may offer a viable option before resorting to total knee arthroplasty.
